Terms of Service

Welcome to MZK Massage. By booking and receiving services from MZK Massage, you agree to the terms outlined below. Please review this document carefully before proceeding with your appointment.

1. About MZK Massage

MZK Massage is a professional mobile massage therapy service based in Phoenix, Arizona. We bring massage services to your home or designated space. Our mission is to provide high-quality, client-centered care in the comfort of your own environment.

2. Services Offered

We currently offer the following massage sessions:

• Durations: 60, 90, and 120 minutes

• Types: Regular Full Body Massage and Deep Tissue Massage

3. Booking & Payment Terms

A $20 non-refundable deposit is required at the time of booking to secure your appointment. This deposit is applied toward the total session cost.

Example: If your massage is $80, your $20 deposit will be applied, and the remaining $60 will be charged automatically after the session is completed.

Payments are processed securely through our verified payment provider, Stripe.

4. Cancellation & Rescheduling Policy

- Cancel/reschedule more than 72 hours before: deposit is refunded or transferred.
- Cancel/reschedule within 72 hours: deposit is forfeited.
- If MZK Massage cancels due to illness or other reasons, the session will be rescheduled at no cost to you.

5. Health & Safety Policy

- Clients must be in good health during their session. Please reschedule if you are ill.
- MZK Massage practitioners will also cancel or reschedule if unwell.
- Clean, sanitized linens are used for each client.
- Notify MZK Massage of any allergies, conditions, or special accommodations needed.
- We do not touch or move client belongings.

6. Client Consent & Confidentiality

All clients must complete a Consent & Liability Waiver Form prior to receiving massage services. Without it, services cannot be rendered.

By consenting, you agree to allow MZK Massage to enter your premises and provide mobile massage services. All client information is strictly confidential and used only to ensure safe, effective services.

7. Code of Conduct

- MZK Massage maintains professional, respectful conduct at all times.
- Clients may dress to their comfort level and will be given privacy to change or reposition.
- Inappropriate behavior will result in session termination and possible future service denial.

8. Age Restrictions

MZK Massage only serves clients who are 18 years of age or older. Services are not available for minors at this time.

9. Mobile Setup Requirements

- Clients must provide a safe, clear area to set up the massage table and equipment.
- The area should be cleared prior to the session start time.
- MZK Massage will not move furniture or handle personal items.

10. Disclaimer & Limitation of Liability

- Massage is not a substitute for medical treatment or diagnosis.
- MZK Massage is not liable for injuries or issues unless caused by gross negligence.
- By booking, you accept these risks and policies.
